Philippine PASSPORT APPLICATION FORMS: New & Renewal
One of the core requirements when applying for a passport, whether for the first time or for renewal, is a duly accomplished application form.
Often, the form is to be filled out online. When trying to schedule an appointment, you will be asked to complete the application form. After submitting the form, you will be asked to pay for the application before they can lock the slot for you. They will hold it for 24 hours. After payment is confirmed, the system will email you a PDF file containing your appointment schedule slip and an electronically accomplished application form. You just need to print both the form and the appointment receipt.

Here are some important notes about the form:
- Print out the form on an A4-size paper.
- You can accomplish the form digitally or by hand. For handwritten forms, make sure you print entries legibly. Use black or blue ink only.
- For fields that do not apply to you, write N/A.
- Minors are defined as those below 18 years of age. and those who are unable to fully take care of themselves.
Bring the accomplished form, together with other documentary requirements, to your appointment at your chosen DFA office.

Hi! What if I had a mistake on the passport application form but I already got through the appointment and they didn’t ask me about it. Is all the information there important? This is for renewal. What should I do? If I call them and raise this concern, willthey reschedule me for another appointment and I would have to pay again? I’m confused. It would be a big help if you would give me n answer immediately. Thank you so much!

Hi Kemarie, di ko alam ung full list of requirements pero you might need to secure a DSWD Clearance muna before applying for a passport for a minor.

Is it necessary to fill up passport renewal form which bears the name of the Philippine embassy in host country e.g. site: Philippine Embassy Qatar? Or I can use a standard renewal form template which bears in the site: xxxxxx.

Helo po, gusto kong mag renew ng passport ko tapos married na ako then my problem is hindi pa lumalabas ung marraige certificate ko galing sa psa, pwede po bang ung marraige license namin galing munispyo then ipapa authenticate ko? Hi Marygrace, valid pa po ba yung nawalang passport? If so, ang alam ko, di need ng NBI clearance. Pero need ng Police Report (English), Affidavit of Loss, and penalty fee.

Hi, I would just like to ask if where can I file for correction of my passport specifically the birth year? instead of 1988 it shows on passport 1998..is there a way i can file for re issuance because i need my passport to take IELTS and NCLEX exam.
